TERMS AND CONDITIONS FOR SALUS CARE PH

Welcome to Salus Care PH (“Salus Care PH”, “we”, “us”, “our”). These Terms and Conditions (“Terms”) govern your use of our website at https://saluscareph.com (the “Site”), your use of our services, the checkout process, the Health Pass Program, and all purchases of prescription drugs, over-the-counter (OTC) medicines, vitamins, supplements, and medical devices.
By accessing the Site, clicking “I Agree”, completing registration, or placing an order, you confirm that you have read, understood, and agree to be bound by these Terms. If you do not agree, discontinue use of the Site.
1. Membership Eligibility and Account Registration
1.1 Age Requirement
You must be at least eighteen (18) years old. If the user is a minor, the account must be managed by a parent or legal guardian.
1.2 Account Accuracy
You agree to provide true, accurate, complete, and current information at all times.
1.3 Account Security
You are solely responsible for maintaining the confidentiality of your account credentials and all activities under your account.
1.4 Account Suspension or Termination
We may suspend or terminate accounts that violate these Terms, involve fraud, or pose security risks.
2. Nature of Services

2.2 Professional Guidance

2.3 Emergency Disclaimer

3. Use of the Site
3.1 Lawful Use

6. Health Pass Program
6.1 Health Pass Requirement
A valid Health Pass may be required to purchase products.
6.2 Fees and Payment
The standard Health Pass is ₱399, valid for one (1) year, unless otherwise stated.
6.3 Activation
Health Pass is activated after successful registration and payment.
6.4 Benefits
May include:
Access to purchase products
Exclusive pricing and promotions
Priority support
6.5 Non-Transferability
Health Pass is personal and non-transferable and may not be shared or resold.
6.6 Renewal
Health Pass must be renewed every twelve (12) months.
6.7 Cancellation and Refund Eligibility
Unless otherwise stated, Health Pass fees are non-refundable.
6.8 Suspension or Termination
Health Pass may be suspended or terminated for fraud, misuse, or violations.
6.9 Changes
We may modify pricing, features, or benefits at any time.
7. Customer Responsibilities
You must:

Follow medical instructions
You acknowledge that misuse or improper use may cause serious health risks.
8. Prescription Requirements
8.1 Requirement
A valid prescription is required for prescription medicines.
8.2 Required Information
Prescription must include:
Patient name
Date issued
Doctor’s name and PRC license number
Medication details
8.3 Validation
Orders will not be processed until validated.
8.4 Right to Decline
We may decline or cancel orders if prescriptions are invalid or suspicious.
9. Product Information and Usage
All product information is for general reference only.
Professional advice and product labels take precedence.
10. Health and Safety Disclaimer
Salus Care PH is not liable for adverse effects resulting from:

11. Purchases, Payments, Returns, and Refunds

A valid government-issued ID may be required.

After delivery, you are responsible for proper storage.
We are not liable for damage due to improper handling.

15. Prohibited Conduct
